didn't pull through...it did form leaves and seemed to be doing OK for a month or so, but then the top where the cut was turned a blackish purple color and it just kept slowly spreading to the rest of the tree

Sorry to hear that. I've recently acquired a Kiyohime maple, so this post has taught me a lot...so if it makes you feel any better, your little Kiyohime didn't die for not

These little guys aren't supposed to be as vigorous as the big ones so maybe taking time and getting to know the tree and it's growth habits and rates will help
What is good for the species not always so much for the cultivars. Kiyohime is perhaps my favorite variety of J. maple but also by far the most difficult to grow well.
