Why holes in ficus trunk?

Big moment this is my first bonsai post, be easy, I’m not completely green but new. Two days ago I rotated the tree and noticed a small hole at the soil line. I researched and learned to clean out and fill to save the tree. I’ve searched online and cannot find specific reasons why this happened to a sufficiently watered healthy tree? From what I’ve found online I should check for root rot? The only reason I can come up Is the I overwatered? I only water when the soil is not moist 2-3 times a day. Please advise.

2-3 times a day seems excessive.
